VISITING SERVICES

We have a number of visiting Allied services available such as Optometrist, Dietician, Podiatrist, diabetes educator, pharmacist and Psychologist.

TELEPHONE ACCESS

GP’s in the Practice may be contacted during normal surgery hours. If the GP is with a patient, a message will be taken and the reception staff will advise you when it is likely that the GP will return your call. Your call will always be put through to the GP in an Emergency.

Douglas Hanly Moir Pathology is based onsite at Dubbo AMS.

PATIENT RECALL AND REMINDER SYSTEM

Patients will be contacted regarding test results requiring a follow-up.

Our Practice is committed to preventative care. You may be included on our reminder system. We may issue you with a reminder notice from time to time offering you preventative health services appropriate to your care. If you do not wish to be part of this system please let your Doctor or reception staff know.

Immunisation information will be passed onto the Australian Childhood Immunisation Register (ACIR) at a National level. 

Information will also be passed onto the NSW Pap Register as appropriate.

Important Information

This Practice is a Bulk-Billing Practice. You will be notified prior to your appointment if there will be a cost (eg. commercial drivers licence, private patient). Veterans will be required to sign the service voucher. There is a potential cost when referred for consultation with medical specialists or other health services.

Please ring 02 68847501 for an appointment. Every effort will be made to accommodate your preferred time and GP. Emergencies will always be given priority and our reception staff will attempt to contact you if there is any unforeseen delay or your GP has been called away. Longer consultation times are available, so please ask our receptionists if you require some extra time. 

If you require an Interpreter or have a hearing impairment and need assistance please advise reception while making appointments and arrangements will be made with an interpreter to assist you.

Your medical record is a confidential document, it is the policy of this practice to maintain security of personal health information at all times and to ensure that this information is only available to authorised staff members.

If you have not been into the Practice for a while we may need to update your details on our computer system. The reason for maintaining updated information is very important in order for us to provide you with a high standard of care.

To ensure that we are in a position to provide the best and most appropriate services and referrals for you or your family we require the following information from you at reception-

  • Your full name
  • Date of Birth
  • Current Contact details including address, phone number and emergency contact details
  • If you identify as Aboriginal or Torres Strait Islander origin
  • Your current Medicare card number
  • Your current Health Care card number

When you make your next appointment please take the time to provide reception staff with your updated details so that your medical records remain current.

We take your concerns, suggestions and complaints seriously. However, if you wish to take the matter further and feel that you need to discuss the matter outside of the surgery there are several options available including the Medical Registration Board, Australian Medical Association (AMA) or Health Care Complaints Commission at:
